Our Case Managers are the heart of our frontline support team—trusted navigators helping Eastie community members and their families overcome the numerous barriers that come with navigating complex systems of support. Whether it’s securing housing, applying for public benefits like SNAP or TAFDC, accessing elder care and disability resources, or understanding healthcare options like Medicaid and Medicare, our Case Managers walk alongside clients every step of the way, acting as a final line of support when other doors have closed.
They support people through some of the most urgent and challenging systems—courts, social services, immigration processes, and healthcare networks—breaking down information and advocating for access. From preparing a resume, connecting someone to trauma-informed care, or referring them to legal help, our innovative and experienced Case Managers ensure no one falls through the cracks.
